Before beginning any work with our Morgan Pavement, all subcontractors are required to complete this form and upload the necessary documentation for review. This process ensures compliance with our contractual, legal, and insurance requirements, and helps us maintain a safe, efficient, and professional working relationship.
Please carefully review and provide the following items:
- Master Service Agreement (MSA): Must be reviewed and signed. If you do not agree with any terms, please submit a redlined version so it can be reviewed and revised as needed. Download the Master Service Agreement Here!
- Current Certificate of Insurance (COI): Must meet the coverage limits outlined in the agreement or the quick reference guide here.
- Current W-9 Form: Must be fully completed and signed.
- Current Contractor’s License: Provide a valid and up-to-date copy.
Incomplete submissions may delay approval and onboarding. Ensure all documents are current, accurate, and clearly legible before submitting.
